The Opportunity

We are seeking a compassionate, senior adult-focused individual to serve as our Senior Services Navigator. The Senior Services Navigator plays a critical role in the Senior Adult department by providing robust services to our senior community. This position works with our Case Management team to identify and navigate direct services and referral services to our senior clients, and they coordinate volunteers and other community partners to assist in distributing services to our senior clients. The Senior Services Navigator is responsible for the daily/seasonal coordination of special services provided to our senior population within our services areas. This position will organize and track services performed and provide support to clients, volunteers, and staff.

Minimum Qualifications

  • Bachelor’s degree in Gerontology, Social Work, or related field
  • 2 years of experience working with older (senior) adults
  • Bilingual (English/Spanish)

Preferred Qualifications

  • Experience working in eldercare or nonprofit
  • Experience working with Salesforce or similar software

Major Responsibilities and Duties

  • Coordinate all aspects of the senior program department and provide creative solutions to senior clientrequests
  • Build effective client relationships, assessing potential barriers, to support, educate and assist senioradult clients
  • Coordinate with volunteers and other support staff to provide appropriate training and materials tomeet the needs of each program
  • Work directly with case management staff to facilitate enrollment and maintenance of senior programs
  • Record all services in our client record keeping software and provide timely reports to superiors

Transportation

  • Receive transportation request calls and respond in a timely manner. Stay up to date with all available transportation resources available to seniors and disabled in the community and provide referrals and connect individuals to those services.

Minor Home Repairs

  • Receive home repair requests and facilitate all aspects of the communication and implementation of the home repair project.

Home Delivered Meals

  • Correspond with vendors and place daily meal orders.
  • Coordinate delivery of meals to clients.

Work Environment

  • The activity for this position requires the employee to work onsite, in the community and be comfortable making home visits to client if necessary
  • Ability to move, transport, position, and lift up to 15 pounds as needed
  • Ability to remain in a stationary position such as standing or sitting for up to 8 hours per day.
  • Ability to walk, move, and/or traverse as needed.

Reporting Relationship

The Senior Services Navigator reports to the Director of Senior Services.

Pay Range

The Senior Services Navigator is an Exempt position with an annual salary range of $45,000 - $50,000.
