The Opportunity
The Housing Case Manager for Older Adults provides comprehensive assessments of senior-aged clients requesting social service assistance. The chosen Case Manager will interview clients to assess needs, identify challenges, and provide financial coaching. The position requires advocacy and collaboration with clients, service providers, and agencies that serve older adults. The Housing Case Manager for Older Adults designs, recommends, implements, and evaluates strategies that help alleviate the aspects of aging, therefore enable older adults to maintain their independence, financial security and good health.
Schedule Commitment
Regular attendance is expected. The Housing Case Manager for Older Adults works in-person from 8:00 a.m. – 5:00 p.m. Monday through Friday. Occasional remote work is available.
Pay Range
This opportunity is exempt and budgeted at $42,000 - $50,000 annually.
Essential Job Functions
- Complete eligibility assessment of all older adults applying to senior programs and verifies all documentation related to client’s eligibility for services and partnership resources
- Complete ongoing annual evaluations and assessments as needed for specific program participants
- Provide programs/services to older adults that enable them to cope with their unique experiences specifically related to older adults
- Serve as a subject matter expert on older adult matters when communicating with clients/families, case workers, and the Metrocrest leadership team
- Investigate the need for and coordinate referrals to older adult-related programs at Metrocrest and throughout the community
- Track the progress of senior programs at Metrocrest Services; prepare agency reports to measure outcomes, identify improvements and implement changes
- Identify partnerships (and funding opportunities with the assistance of the Development group) to promote relationship-building and service delivery with other organizations, agencies, and/or individuals that serve the needs of senior adults
- Serve as an advocate and liaison for older adult matters to the Metrocrest leadership team, Senior Programs, and the local community
- Schedule and conduct individual client appointments using motivational interview techniques and assessment skills in determine barriers and level of service need based on situation
- Develop an older adult service plan which is client focused
- Evaluate older adults’ potential needs that can be fulfilled by programs we provide including but not limited to home delivered meals, transportation, pantry delivery, and/or minor home repair
- Prepare and submit all service related documentation for program participants both within housing and senior departments
- Maintain accurate and confidential records on each client for assigned case load as well as entering all client information into appropriate program database
- Maintain program policies and guidelines for all clients
- Maintain open communication at attend ongoing meetings with other program staff including with Senior Programs
- Set high standards for professional, technical, personal conduct and competence in multiple software applications (Excel, Word, PowerPoint, and Database entry)
Education, Experience, & Competencies
- Bachelor’s or Master’s degree in Gerontology, Social Work or related field
- Minimum three to five years of experience working with older adults
- Demonstrated in-depth knowledge and understanding of the unique barriers and challenges facing senior adults
- Demonstrated ability to navigate all community resources including benefits and services unique to older adults
- Knowledge and effective execution of de-escalation techniques and conflict resolution
- Knowledge of Salesforce, HMIS or related client management databases (preferred)
- Ability to work with a diverse client base and a multi-disciplinary team
- Ability to establish and maintain effective working relationships with other employees and the public
- Excellent communication, organizational and time management skills with ability to perform duties with minimal supervision if necessary
- Bilingual English/Spanish (preferred)
Leadership Relationship
The Housing Case Manager for Older Adults reports directly to the Housing Manager.
