Elevate North Texas (Elevate NTX) provides immediate, short-term housing and personalized case management for youth ages 18-24. Since we opened our door in 2021, we have offered a welcoming, affirming environment to youth experiencing homelessness. Through a holistic approach involving support, case management, and job placement assistance, Elevate NTX equips youth with the necessary resources to break the cycle of homelessness. Elevate NTX provides youth in its programs with hotel stays for immediate, short-term housing, reunification assistance with welcoming and affirming family or friends, and diversion to longer-term housing options.

Position Summary:

The Transitional Housing Case Manager (TH CM) will work closely with youth, other ENTX staff, and community partners to provide transitional housing and wrap around supportive services, assisting the program participants to move to permanent housing as quickly as possible. The TH CM is responsible for the case management of the youth participating in Elevate NTX’s Transitional Housing Program. Successful case management while the youth is in the Transitional Housing Program is a key component of Elevate NTX’s mission and critical to ending youth homelessness. The person who fills this position must have a strong working knowledge of how social service systems operate and possess the empathy required to understand and relate to individuals who work within this field and the youth who interact with it. The TH CM must work respectfully with people in crisis who may have multiple complex needs, including but not limited to domestic violence, alcohol and drug addiction, mental health, poverty, disability, and homelessness. The TH CM makes referrals to partner agencies and tracks outcomes related to each youth’s participation in the Transitional Housing Program.

ESSENTIAL FUNCTIONS

  • Assist youth who are experiencing homelessness with obtaining or returning to a positive housing destination.
  • Partner with youth using Housing Problem Solving to develop creative housing solutions unique to the individual, such as family mediation, roommate agreements, rental agreements, repayment agreements, verbal and/or written agreements between family members, conversations with probation officers, landlords.
  • Implement core principles and evidence-based practices including, but not limited to: Positive Youth Development, Trauma Informed Care, Right Response, Housing First, Youth Mental Health First Aid, LGBTQ Cultural Competence.
  • Act as a liaison between youth experiencing a housing crisis and supportive services, landlords, and community agencies, including Joint RR (Rapid Rehousing) & PSH (Permanent Supportive Housing).
  • Develop a housing plan with each youth and coordinate with appropriate programs.
  • Collaborate and coordinate educational, vocational, employment, and career development and exploration activities, including making referrals to the vocational advocate when appropriate.
  • Provides support and direction for youth as they transition from homelessness to a positive housing destination.
  • Build high-quality relationships with youth participants to understand the personal experiences of experiencing homelessness.
  • Enter, update, and maintain data in ENTX’s case management and other required systems (e.g., Homeless Management Information System (HMIS), accurately maintaining all required records, reports, and files.
  • Work as a team with ENTX and shelter staff, volunteers, and community partners.
  • Create and sustain community partnerships to ensure mainstream resources are being utilized by youth.
  • Participate in the Point in Time Count.
  • Other duties as reasonably assigned by supervisor or the Executive Director.

Minimum Qualifications:

  • A bachelor’s degree in social work or a related field or an equivalent combination of education, training, and experience.
  • Minimum of one year of relevant experience, including crisis de-escalation skills, motivational interviewing and Stages of Change, Harm Reduction, Housing First, and Trauma Informed Care.
  • Knowledge of issues related to youth homelessness, housing opportunities and programs for homeless youth, and other social services.
  • Effective communication skills, both oral and written. Ability to establish and maintain effective working relationships with other employees, volunteers, community partners, and the public.
  • Ability to exercise sound judgment; maintain detailed, organized records; work independently; and handle multiple tasks simultaneously.
  • Intermediate proficiency in Word, Excel, Outlook, PowerPoint, Publisher, and other commonly used software.
  • Knowledge of, and passion for, local housing and poverty issues in North Texas.
  • Valid Texas driver’s license, a good driving record and legally required auto insurance.
  • CPR and First Aid Certification must be current or obtained within 60 days of hire.

Preferred Qualifications:

  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
  • Bilingual (English/Spanish).

Office Location: Far North Dallas

Travel: Frequent travel in Dallas & Collin Counties required to meet with youth.
