Job Opening:

Recovery Resource Council is seeking a full-time Housing Case Manager to provide intensive case management to individuals in Shelter4hope, a permanent supportive housing program for chronically homeless individuals with disabilities, including physical disabilities, chronic illness, mental illness, development disabilities and substance abuse disorders. Shelter4hope operates as a Housing First program in Fort Worth, Texas.  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required.  Licensed Chemical Dependency Counselor (LCDC), approved Counselor Intern (CI), or Qualified Credentialed Counselor (QCC) that is “exempt” preferred.  Bachelor’s degree in social services with case manager experience will be considered.

 Duties and responsibilities:

Staff hired for this position will assess and determine eligibility of participants for Shelter4hope housing intensive case management Program. Assists chronically homeless individuals with completing Housing Authority applications. Communicate with Housing Authority to determine what documents client needs and assist in retrieving those documents and sending them to appropriate Authority. Accompany client to Voucher briefing to assist if needed. Once a client is issued a voucher, assist client with securing a home, including assistance with finding an apartment unit, ensuring the unit meets housing quality standards, and accessing resources to begin a household. Provide contact and support necessary for the client to remain in housing. Provide on-going case management once a week including home visits to participants of the program. Develop mutually agreed upon, individually designed housing plan with the participant. Follow up on progress and help the participant overcome any barriers to success by finding resources, providing referrals, or offering education and support depending on the issue at hand. Facilitate monthly support group. Support groups can be virtual if necessary. Enter participant data into HMIS as required. Provide complete documentation of all services provided, including program eligibility verification, consents, client interviews, progress notes and service plan updates. Provide information as necessary for collaboration with funding sources and evaluation purposes. Establish working relationships with participating apartment complexes for the benefit of the participants and property owners.
