The Homeless Case Manager Project Coordinator will provide case management for homeless children and families at three Tarrant County shelters. This position will also serve as a liaison with partner and community resource agencies and assess and report on the needs of homeless children and families.

  • Provides case management for homeless children and families at three Tarrant County shelters:
    • Prioritizes work assignments and manages time wisely, requiring little supervision in this area
    • Communicates effectively with medical professionals and client families
    • Works with Cook Children’s RN case manager to provide comprehensive case management services for clients and their families
  • Serves as a liaison with partner and community resource agencies:
    • Attends meetings, workshops, and trainings to build relationships with agencies in the community
    • Maintains relationship with other agencies and service providers
  • Assesses children’s needs and family functioning and provides crisis intervention, information and referral, problem-solving, and case management in diverse family and cultural contexts:
    • Works with shelter staff, case managers, and partner agencies to ensure needs of homeless children and families are met
    • Meets with shelters’ children’s staff to discuss current needs of clients
    • Attends shelter community group meetings and designates time to spend at each shelter, to be available to clients
  • Conducts assessment of overall needs of homeless children and families in Tarrant County:
    • Exhibits strong psychosocial assessment and planning skills
    • Works with peers and supervisors in establishing appropriate plans and assessment process
    • Participates in regular individual and/or group supervision
  • Conducts life-skills groups for homeless children at shelters:
    • Facilitates outside agencies and partners in leading groups with the children
    • Engages children in topics and activities that are enjoyable and educational
  • Coordinates and schedules Parent Education Groups at 3 area homeless shelters:
    • Facilitates outside agencies and partners in leading groups with parents
  • Leads coordination efforts among other case managers and staff of participating service providers, medical and legal professionals, and the Tarrant County Homeless Coalition
  • Completes monthly and quarterly reports
    • Maintains client files and current client numbers in agency’s database
    • Enters service hours complete into HMIS database
    • Complies with other reporting responsibilities as needed

Experience Requirements

  • Master’s degree in Social Work + five (5) years’ experience
  • Must be licensed by the Texas State Board of Social Work Examiners
  • Must have experience working with and understanding the needs of vulnerable populations (including         homeless, substance abusers, persons with physical or mental development disabilities, victims of domestic violence/sexual assault/abuse, etc.)
